Division Specific Forms and Information

Please print, sign and return the required forms (forms can be found via the division links below) to the attention of the LS, MS, US or Athletics Department, no later than Friday, August 10, 2012.

Forms to be printed, signed and returned are marked by an (*).

For Parents with Children Entering Lower School Grades (K-5)

For Parents with Children Entering Middle School Grades (6-8)

For Parents with Children Entering Upper School Grades (9-12)

For Parents with Children wishing to participate in US Athletics

If not, please complete the following forms online, via the following link: 

Mandatory online re-enrollment forms (including: Disaster Relief Information, Emergency Contact and Medical Authorization Form, Assumption of Risk, Release, Indemnity and Consent to Participate Agreement and Vehicle Registration Form).

If you don't have your User ID or password you can use the "forgotten password" link available on the forms website to retrieve it. If you need additional support please contact the Tech Department: support@headroyce.org
